Pay-4-Performance & Culture Change

Interest in state and federal efforts to incentivize quality is growing. This session will explore two efforts to lay the groundwork for a pay-for-performance system that rewards providers across the long-term care spectrum for person-directed training. The North Carolina NOVA program involves creating a special voluntary program whereby nursing homes, assisted living residences and home care agencies can qualify for a special licensure designation if they meet a comprehensive set of workplace culture change criteria. In Kansas, an actual pay-for-performance system is in place as it relates to Medicaid reimbursement in nursing homes. Homes now being rewarded with higher per diem rates are shown to perform better on a range of indicators associated with person-directed care.
From Practice to Policy
Leveraging State and Local Policy Resources to Change a State’s Caregiving Culture

This session explores ways to build upon the success of organizations that have been early adopters of culture change. Guides will explain how these pioneers have planted the seeds that can eventually transform an entire state’s caregiving culture. The session draws on real examples from four states (MA, NC, PA, and MI) where public and private financing has been harnessed to further culture change initiatives and where regulatory expectations relating to the long-term care sector have been reformed. This session is designed for culture-change champions who want to learn how to accelerate the pace of culture change in their states. Discussion will include leveraging of policy opportunities that increase funding for these efforts or that change regulations which impede culture change practices in long-term care settings.
Advancing Resident and Worker Choice
Through Grassroots & Media Advocacy

Implementing a new culture of long-term care calls for a renewed emphasis on advocacy at multiple levels. In this session, innovative leaders from Florida and Pennsylvania will share strategies for using media and grassroots advocacy to accelerate change. Mary Ellen Early will share her experiences with a Florida coalition that successfully changed the ways in which existing state regulations were incompatable with resident-directed choice. Francine Fineman will share how direct care workers have come together on a grassroots level to make their voices heard in the culture change dialogue in Pennsylvania.
Turning Civil Money Penalties
into Culture Change Gold

Resources to implement culture change iniatives are available from a range of sources, and a potentially valuable one is the Civil Money Penalties collected as part of the state oversight process. This session will explore the results of a project supported by the Commonwealth Fund to assess how states are using Civil Money Penalties to advance person-directed care practices. Attendees will learn strategies for finding resources to implement and sustain culture change practices.
