Here's a quick update on Connecticut's coalition work:
Connecticut providers and consumers have been working hard to formalize a coalition in CT dedicated to brining about awareness and education of culture change in LTC. The coalition has met twice and plans to meet again in June (email NHQI@qualidigm.org if interested).
The coalition's mission is to drive positive changes in LTC from institutional models of care to person-centered environments. Our coalition's vision is to promote development of person-centered environments where individuals live, work and grow in nurturing, respectful communities that honor personal choice and diverse lifestyles.
We accomplish this by supporting culture change through: education, research, sharing, advocating and collaborative relationships.
