Culture Change Efforts in Arizona

On behalf of Health Services Advisory Group(HSAG), Arizona's Quality Improvement Organization (QIO), I want to take this opportunity to provide an update of recent culture change efforts in Arizona.

As many of you are probably aware, Arizona was one of the 21 states involved in the Improving Nursing Home Culture (INHC) Pilot Collaborative conducted by Quality Partners of Rhode Island. Here in AZ, we had 8 nursing homes (NHs) participate from October 2004 to July 2005.

Our interventions included 3 learning sessions and an Outcomes Congress. Between learning sessions, HSAG conducted on-site visitations to the participating NHs, helping with change ideas. In referencing these materials, it is only appropriate to thank Quality Partners of Rhode Island as well as Cathie Brady and Barbara Frank for their guidance during the Collaborative. Also, I want to thank John Linda and Kathleen Collins-Pagels of the Arizona Health Care Association (AHCA) for connecting our AZ Collaborative with Steve Shields during the AHCA 2005 Spring Conference. This was a wonderful opportunity to hear from a real pioneer of NH culture change.

In the 8th Scope of Work, HSAG is using culture change methodology in working with participating NHs to improve workplace practice, care practices, and the environment, drawing from the successes and lessons learned during our Arizona pilot.

Yesterday (April 26, 2006), I had the opportunity to present at the Arizona Association of Homes and Housing for the Aging (AzAHA) annual meeting on "Getting Started on the Culture Change Journey". Although our state has made significant steps toward person centered care, I believe, as a whole, AZ is still in an "awakening" or "fertilizing the soil" stage.

In terms of coalitions or organized culture change efforts, Health Services Advisory Group hosts the Arizona Quality Counts Partnership (AQCP). The AQCP meets bi-monthly and includes, but is not limited to NH and home health agency (HHA) health care associations, the State Survey Agency (SSA), Medicaid Program Contractors, Governor's Advisory Council of Aging representation, managed care plans, ombudsman office, etc.. During these meetings, each stakeholder shares initiatives within their organization. Culture change in NHs has been a leading topic and embraced by the AQCP. From the AQCP discussions on NH culture change, the NH SSA decided to train all its surveyors on culture change initiatives, thereby supporting this effort.
